Prior to your first visit, there are several helpful steps to take to minimize time delays and insurance reimbursment difficulties.

• Print out the Insurance Verification Form. Fill out the Subscriber and Patient information. Call your insurance company prior to your visit, and find out and note the authorization number (if any); your deductable; your co-payment amount; how many visits you are eligible for, and the address to which to submit claims.
• Print out the New Patient Registration Form and fill it out, and bring it with you to your first visit.
• If you are making an appoinment with one of your psychiatrists, you will be asked to download the Cancellation Policy-Psychiatrists and the New Patient Registration Form and fax those to our office, at 310-257-5753.
• Read the HIPPA information, and download and sign the signature page.
• In order to coordinaate care, we prefer to communicate with your Primary Care Physician. Print out the Health Care Coordination Form, fill out the shaded area with you doctor's name, address, and fax number, and bring it to your first visit. If you do not want your Primary Care Physician notified, do not fill out the form, and let your provider know at your first visit.
